I received a bill, in January 2011, from a doctor for a service he provided in March 2010. I wrote to UnitedHealthcare that same day asking why the claim was denied.

I provided UnitedHealth with copies of four similiar procedures which were done by the same doctor. Why was one of the three claims denied by UnitedHealthcare, yet the others were not?

To this day they have not provided me with a reason or medical explaination. All I have to go on are the records from the doctor’s office showing that they received a “denial of claim” notice from UnitedHealthcare in December 2010.
